What Is Sciatica?
Sciatica isn't actually a condition itself; it's a symptom. The NHS defines it as pain caused by irritation or compression of the sciatic nerve, which runs from your lower back down through your buttocks and leg.
The telltale signs? Sharp, shooting pain that travels from your lower back down the back or side of your leg, sometimes reaching your foot. You might also experience burning sensations, electric shock-like pain, pins and needles, or numbness. Some people describe it as feeling like "running water" down their leg; that's your irritated nerves sending mixed signals.
Most sciatica stems from a slipped disc pressing against nerve roots, though spinal stenosis (narrowing of the spinal canal) or inflammation around the nerve can also trigger symptoms. The good news? According to health guidance, most sciatica improves within 6-12 weeks, and treatment can help speed this process.
How Acupuncture May Help Sciatica
Traditional acupuncture works on multiple levels to address sciatic pain. The fine needles help modulate pain signals travelling to your brain, essentially turning down the volume on that sharp, shooting sensation. They also encourage your body to release natural pain-relieving endorphins and anti-inflammatory compounds.
But there's more to it than just pain relief. Acupuncture can help relax the tight, spasmed muscles often associated with sciatica, particularly around your lower back and piriformis muscle. The treatment also improves local blood circulation, helping reduce inflammation around irritated nerve roots.
A comprehensive 2023 review analysing 28 studies with 2,707 participants found acupuncture significantly more effective than conventional pain medication for sciatica. The research showed people receiving acupuncture had better pain scores and improved functional outcomes.
What about timing? Some people notice subtle changes after just one session, such as reduced morning stiffness or the ability to sit for longer periods. More substantial improvements typically develop over 3-4 treatments. A typical treatment plan involves 6-8 sessions over 4-6 weeks, though this varies based on how long you've had symptoms and their severity.
Remember, there's no magic formula. Each person's sciatica journey is different, which is why we tailor every treatment plan to your specific symptoms, triggers, and lifestyle.
Your First Visit: What to Expect
Assessment comes first. William will spend time understanding your pain, when it started, what makes it worse, and how it affects your daily routine. He'll ask about your work setup (desk job? manual labour?), exercise habits, and any previous treatments you've tried.
You'll then have a gentle physical examination. This might include checking how far you can bend forward, testing muscle strength in your legs, or identifying tender spots. Don't worry, we won't ask you to do anything that significantly increases your pain.
The treatment itself involves strategically placed hair-thin needles, typically in your lower back, buttocks, and legs. Many people are surprised by how comfortable it feels; these aren't like injection needles. You might feel a slight tingling or dull ache when the needles go in, which is actually a good sign that we're hitting the right spots.
We often combine acupuncture with electro-stimulation (tiny electrical pulses between needles) for enhanced pain relief, and sometimes add cupping or heat therapy. All needles are single-use and sterile; we follow strict hygiene protocols with fresh air ventilation and individualised treatment rooms.
After your session, some people feel immediately lighter or more mobile. Others might feel slightly drowsy; that's your nervous system rebalancing. We'll provide you with specific aftercare advice, including gentle stretches and guidance on using heat/ice at home.
Who It's For (and When It's Not)
Acupuncture works particularly well for:
Sciatica caused by a disc herniation or bulging discs
Piriformis syndrome (when your piriformis muscle irritates the sciatic nerve)
Nerve root irritation from inflammation
People looking for drug-free pain management
Those who haven't found lasting relief with conventional treatments alone
We take extra care with pregnancy-related sciatica; specific acupuncture points are avoided, but treatment can still be safely provided with appropriate modifications.
However, acupuncture isn't suitable if you:
Have an active infection near the treatment area
Experience those red flag symptoms mentioned earlier (loss of bladder/bowel control, etc.)
Have certain bleeding disorders
If you're experiencing severe weakness in your leg, worsening numbness, or your pain is getting progressively worse despite initial treatments, we'd recommend getting assessed by your GP or a spinal specialist before beginning acupuncture.
FAQs
How many acupuncture sessions are needed for sciatica?
Most people notice meaningful changes within 3-4 sessions, with a typical course involving 6-8 treatments over 4-6 weeks. Recent research used 10 sessions over 4 weeks and found significant improvements. The exact number depends on how long you've had symptoms. Acute sciatica (under 6 weeks) often responds faster than chronic cases.
How quickly could I feel relief?
Some people feel subtle improvements within 24-48 hours of their first session, perhaps they can sit longer or sleep more comfortably. More noticeable pain reduction typically develops after 2-3 treatments. However, everyone's different. Chronic sciatica that's been present for months may take longer to respond than recent flare-ups.
Is acupuncture safe if I'm on pain medication?
Generally, yes, though we need to know exactly what you're taking. Acupuncture can actually complement conventional pain relief; many people find they need less medication as their symptoms improve.
Will it help if my sciatica comes from a herniated disc?
Research suggests yes. The 2024 JAMA study specifically looked at chronic sciatica from herniated discs and found significant improvements with acupuncture. While we can't "fix" the disc itself, acupuncture can help reduce inflammation, relax muscle spasm, and change how your nervous system processes pain signals from that area.
Can acupuncture be combined with physio or chiropractic?
Absolutely. In fact, many of our patients see the best results when combining acupuncture with other treatments. Acupuncture can help reduce your pain levels, making physiotherapy exercises more tolerable. We're happy to coordinate with your other healthcare providers.
How much does treatment cost?
Each session costs €95 and includes acupuncture, electro-acupuncture if appropriate, cupping, red light therapy, and herbal consultation. Most private health insurers (VHI, Laya, Irish Life) provide coverage, and we'll provide you with receipts to submit for reimbursement.
What should I wear to appointments?
Wear loose, comfortable clothing that can be easily rolled up. We typically need access to your lower back, buttocks, and legs, so tracksuit bottoms or loose trousers work well. Avoid tight jeans or fitted clothing that's difficult to adjust.
Is there anything I should avoid before treatment?
Don't arrive on an empty stomach, but also avoid large meals within 2 hours of your appointment. You will not be seen if you have consumed alcohol or illegal drugs. If you're having a particularly bad pain day, still come in; these are often when treatment is most beneficial.
Ready to Start?
Sciatica flare-ups often respond best when treated early, before pain patterns become deeply established. The longer nerve irritation persists, the more your nervous system can become sensitised to pain signals.
You don't have to live with shooting leg pain, sleepless nights, or the frustration of being unable to sit comfortably. Our evidence-based acupuncture approach provides a drug-free way to alleviate your symptoms and enhance mobility.
